2. Your actual Date of Birth

In this format:  21st March 1999

**In some countries the date of birth is given as the day registered; this is incorrect and requires the actual date you are born**

 

3.  Current country and State of Residence **this is to establish time zones**

 

4.  Please let me know if you have had a session with me before, as I will have an archived copy of your chart and returning clients for a Number Analysis receive a reduced rate.
